Important
A registered attendee will not be admitted to the conference unless they have a PAID registration. An
attendee can pay at check-in with a check or credit card. Cash will not be accepted.
Conference Registration Payment Options
At the end of the registration process, you will be given four payment type options. You must select one of those options in order to complete your registration.
- Credit Card Online
- We accept Visa, Mastercard, or Discover only; we regret that we are unable to accept American Express.
- If you select this payment type, have your credit card ready; and be sure to enter the zip code for the mailing address of the card owner; this may not match your zip code on your registration.
- If you do not select this payment type at registration, you can still pay your fee later by credit card; the KCM can email a link to pay any invoice by credit card at any time.
- Check or Money Order
- Select this option if you know you want to mail a check or money order to the KCM to pay your invoice.
- Checks should be made out to Kentucky Center for Mathematics.

- Bill Me (Print Invoice)
- This is the best choice if you are unsure how your registration fee will be paid. After you select this option, your fee can be paid later by credit card or check; and, we can add a PO# to your registration at any time.
- Purchase Order Payment
- Several Kentucky school districts require that their educators obtain a Purchase Order before they will pay any invoice. Please check with your school, district or organization if they require you to get a Purchase Order before you register.
- If you do not have the PO# at the time you register, but are in the process of getting one, please put "pending" in the Purchase Order Number box at the bottom of page 1; and then give that PO# to the KCM

- Please note that a Purchase Order does not pay an invoice; it shows the expense was pre-approved and acts as a promise that the registration fee will be paid by check or credit card by your school, district or organization.
W-9 (Request for Taxpayer Identification Number and Certification): Please note that the KCM is housed at Northern Kentucky University (NKU). We use NKU's W-9 form.

Cancellation Policy
All registration fees will not be waived and are non-refundable after February 22, 2026. Please read the full Cancellation Policy for what to do if you cannot attend
the conference after you have registered.
